Community and Aged Services

The main Community and Aged Services administration is at Greenwich Hospital in Sydney’s north with a base at Braeside Hospital for services in Sydney’s south west and west.

We offer a range of services, delivered in home or in Day Centres which aim to provide a continuity of care in the form of assistance and flexible support for the frail aged and younger people with disabilities and their carers, to support their independence.

Lower North Shore Community Options Tel: 9903 8366
Lower North Shore Community Options provides case management to support frail aged people, people with disabilities and their carers living in the local government areas of North Sydney, Mosman, Lane Cove and Willoughby.  The case manager coordinates a range of services to support clients with complex needs, to assist their independence and to prevent premature or inappropriate placement in residential aged care.

Transitional Care Program Tel: 9903 8366

Transpac is home based therapy program for frail aged people who wish to return home, post discharge from hospital.  We administer the program covering the Lower North Shore, Ryde, Hunters Hill and Gladesville.  The program includes Physiotherapy, Occupational Therapy and a range of home support services for an up to three-month period

Compacks is six week, post discharge program for people in need of short-term home support services. Case Management is also involved to link clients with on going services and supports as needed as needed.

Day Centre Services

Tom O'Neill Dementia Day Care Centre

The Tom O'Neill Centre provides dementia specific services within a Day Care setting.  The service provides direct care for people with dementia and respite for their carers.
Monday to Friday - for clients with moderate to severe dementia.
Saturday - 'Horizons' Program - for people with early age/early stage dementia who are physically fit and active. 
Friday Group - a social activity program once a month, to support aged people with a mental health diagnosis.
Carer Support Group - quarterly meeting held at the Tom O'Neill Centre to provide a forum of support, information and education to support carers of a person with dementia.

The Greenwich Day Centre on the Greenwich Hospital campus provides socialisation for frail aged people living at home who have difficulty being involved in other community activities.

Respite Programs

The respite program for people residing in the Lower North Shore area provides support for people with dementia who are not able to attend a Day Centre program, and respite for their carers.

A program to support carers of people with a diagnosis of dementia with related challenging behaviours who reside in the Lower North Shore or Ryde Hunters Hill areas.
